Academic Services

The office of Academic Support Services creates accommodation plans for students with disabilities. We advise students with disabilities to register with our office as soon as they are accepted by the College. Registering for services involves completing the application package below and mailing it directly to the Powell Resource Center along with the required documentation. You can click on the links below to fill out the information online or you can request a complete application package mailed to you by calling 276.944.6145.

Individual Application Package Forms
You should review the first two forms completely before filling ou the Dissability Disclosure form and the Disability Accomadations Request Form. Both forms are required for most students.

Registered Students with Accommodations
The following section provides a list of forms required only for students with disabilities who are registered with Academic Support Services.

Requesting Books on Tape
Qualified students can use this link to request textbooks in alternative formats. To qualify the student must have a documented learning disability or visual impairment. In addition, the student must be registered with the office of Academic Support Services.

Signature Forms
All students with accomodations are required to meet with their professors, provide them with a copy of their accommodation plan, and return a signed copy of the signature form to the Powell Resource Center. Professors are not required to provide accommodations to students until they have received a copy of their accommodation plan.
