Disability Services
The office of Academic Support Services creates accommodation plans for students with disabilities. Students with documented disabilities are encouraged to register with Academic Support Services as soon as they are accepted to the College.
Registering with Academic Support Services involves submitting appropriate documentation and the Disabilities Disclosure Form as well as having an interview with the Director. In order to receive accommodations, you must register with Academic Support Services and submit the Disability Accommodations Request Form. You can download the forms below or request an application package to be mailed to you by calling 276.944.6145.

You should review the first two forms completely before filling out the Disability Disclosure Form and the Disability Accommodations Request Form.
Requesting Books on Tape
Qualified students can use this link to request textbooks in alternative formats. To qualify the student must have a documented learning disability or visual impairment. In addition, the student must be registered with the office of Academic Support Services.
Signature Forms
All students with accommodations are required to meet with their professors, provide professors with a copy of the accommodation plan, and return a signed copy of the signature form to the Powell Resource Center. Professors are not required to provide accommodations to students until they have received a copy of the accommodation plan.
