2013 Summer Orientation - Pre-Enrollment Forms and Documents
Please review each document carefully. Confirmation and schedule of summer orientation will be sent out beginning May 1 to your Emory & Henry email account. Forms should be returned to the E&H Admissions Office, PO Box 947, Emory, VA 24327 before you attend Summer Orientation unless otherwise designated. If you have questions, please contact the Office of Orientation Programs at 276-944-6670.

Sickle Cell Information
Student Health Forms - Remember to submit your “E&H Immunization Record and Medical Information Form” before August 1, 2013. Please read and follow directions carefully making sure all blanks/boxes are complete with applicable signatures. Please attach a copy of the front and back of your insurance card. VARSITY ATHLETES must attach proof (lab report) of sickle cell trait testing or sign the waiver located on the athletic website. If you have any questions about Health Center requirements, please call 276-944-6219. The Health Center will be open from 8:00 – 4:00 through May 31. Messages left during June and July will be returned as soon as possible.
