Prospective Students
Application for Admission (PDF) -- prospective students can submit an application, or apply online. The $30 application fee is waived for students who apply online.

Transcript Request Form (PDF) -- use this form to request official transcripts and test scores from your high school.

Admitted Students
Residence Life Data Sheet (PDF) -- for on-campus residents.

Residence Life Residence Requirement Waiver (PDF) -- for students who will be living off-campus.

Medical Information Form (PDF) -- to be completed by the student and his or her physician. This document must be returned to the Dean of Students Office

Transfer Students
Dean's Certificate for Transfer Students (PDF)
Must be completed by the Dean of Students or corresponding official from the last college attended by the applicant.

Note: Transfer students are required to supply transcripts from all colleges previously attended. High school transcripts are required if students have completed less than 26 semester hours. Click here for more information.
